Transition Coordination Services
Premier Social Services assist Participants in transiting back into the Community. Our Coordinators help to Participant establish a Support System to include family members, providers and other resources while continuing to focus on their health and overall, well-being.
Care Coordination Services
Premier Social Services supports Participants in the coordination of care that is appropriate and safe. Through a Person-Centered approach, Participants needs are conveyed to the necessary resources to assist in the delivery of high quality and effective healthcare services.
